Completing your Redundancy Payments claim form (RP1)

Questions 1 – 16     Personal details
It is important that your National Insurance number is entered correctly as well as your name, address and bank details.

Questions 17 – 21  Employer details
Enter the name of your ex-employer, noting in question 21 c) that the employer is insolvent.

Questions 22 – 24  Details of Insolvency Practitioner
If this section is not already completed, enter the details of the Insolvency Practitioner handling the insolvency.

Questions 25 – 45  Employment details
Question 28 asks for the date you started working for this employer. If you have continuity of employment from a previous employer, enter this earlier start date.
Usually the dates for questions 29 and 30 will be the same. Often the due notice will not have been able to be given, so question 32 will have the same date.

Questions 46 – 54  Business transfer details
If you have had a new job this section will need to be completed.
Questions 49 to 54 only need to be answered if the new job is with an employer connected with your former employer.

Questions 55 – 57  Redundancy Pay claim
If you are entitled to Redundancy Pay tick the “Yes” box in question 55. Generally, questions 56 and 57 are not relevant.

Questions 58 – 73  Wages, Holiday Pay, Notice Pay Claim
For questions 58 to 68 enter details of your holiday pay arrangement. For the answer to question 57 you may wish to check the “Holiday Pay Entitlement Calculation” page.
For question 69, enter any specific dates that you have worked for which you have not received payment.

Declaration

Be sure to sign and date the form before returning it.

Usually the form should be returned to the Insolvency Practitioner for onward submission to the Redundancy Payments Office.
